Service Options Using Resources in a Community Environment (SOURCE)

SOURCE operates under authority of the Georgia Elderly and Disabled 1915c Medicaid Waiver and provides Home and Community-Based Services to elderly and physically disabled people who meet the Intermediate Nursing Home Level of Care.

Health Force of Georgia is a Georgia Approved Provider

The Health Force of Georgia is a participating provider of at-home care services for persons eligible for the (SOURCE) waiver in Georgia. Because the Health Force of Georgia is paid directly by the State of Georgia, individuals who qualify for the (SOURCE) program may receive home care or skilled nursing care services at no cost.

In-home care services may be provided in a variety of ways. Here are some examples:

  • Personal Care
  • Meal Preparation
  • Light Housekeeping
  • Wound Care
  • Skilled Nursing Care
  • Respite Care
  • Medical Management
  • Care Coordination – With the client’s regular doctor or medical provider

Who is Eligible for Georgia SOURCE?

In Georgia, to be eligible for the program, you must be 65 years old or older and have a chronic medical condition, or you must be under 65 years old and disabled. Individuals must be covered by Medicaid and fulfill other eligibility criteria in order to be eligible for (SOURCE).
The following are examples of common conditions:

  • Asthma or other respiratory difficulties
  • Diabetes
  • Arthritis (rheumatoid and osteoporosis)
  • Stroke
  • High Blood Pressure
  • Amputations
  • Alzheimer’s Disease
  • Spinal Cord Injury
