Astin’s Home Care Specialty Program
Our specialty programs are a part of our in-home care service and are focused on proving care and support to clients with chronic illnesses like diabetes, cancer, Alzheimer’s, and other special conditions. The main objective behind these programs is to improve the quality of life for all involved, i.e., the client and their family.
For the client, we work together with their family and or healthcare provider/s to create a personalized program that supports their desire to remain at home while ensuring their in-home care needs are met efficiently, compassionately, and respectfully. For the family, we liberate them to go and attend to their everyday responsibilities fully assured that their loved one is in the best hands possible.
How The Programs Work:
A specialty program begins with the client or family contacting us through a phone call or an email. Our contact details have been provided at the end of this page.
After this initial contact, our care coordinator sets up an in-home visit to the client’s home, where a Registered Nurse makes a thorough assessment of the client’s needs. We also make use of this initial home visit to interact with the client and the family as a whole, and also to answer any questions they may have concerning the program and our agency.
Next, we collaborate with the client and or family to develop a customized Service Plan based on the client’s needs. We particularly consider the specific needs of the client when assigning a caregiver or an in-home care aide to them. For example, if the client’s special condition is Alzheimer’s or diabetes, or both, he or she gets matched up with a caregiver with specialized training in Alzheimer’s and/or Diabetes care.
Services Offered Through Our Specialty Programs:
The services offered through our Specialty Programs team are all medical and or non-medical, and they include:
Skilled Nursing: includes medication administration, wound care, education etc.
Personal care – includes tasks like toileting, grooming, nail, and haircare, etc.
Companion care – includes socializing and mental stimulation tasks like reading, taking walks, talking, listening to music, playing board games and card, etc.
Homemaker services – include homemaking tasks like dusting, vacuuming, washing the dishes, doing laundry, and other light housekeeping tasks.
Non-medical transportation to and from medical appointments, social engagements, and others.
Shopping and errands.
For clients who require a highly more complex level of skilled care, Astin Home Care can collaborate with a reputable home health agency to ensure that the medical needs of the patient are met as per their healthcare providers’ instructions.
